Nurse Health Coach

Promise Community Health Center seeks a full-time Nurse Health Coach to join its growing health-care team. Promise seeks applicants who are passionate about partnering with patients and making our community a healthier place.


Responsibilities include:

  • Serving as a resource to individuals, professionals, case workers and the broader community on health topics – teaching people to make good health decisions, incorporate healthy activities into their lives and avoid harmful behaviors;

  • Meeting one-on-one with patients, the public and at-risk populations, utilizing effective teaching methods to ensure understanding of health topics being taught;

  • Serving as a care manager for patients with one or more chronic conditions, partnering with them to identify health goals, following up with them regularly and communicating with patient’s care team members;

  • Utilizing motivational interviewing to empower patients to make changes in their self-care;

  • Making home visits if and when needed to ensure that the home setting is conducive to patients continuing a home program and to follow-up with lifestyle changes and teaching done in the health center;

  • Organizing and leading group classes to address chronic health issues and providing support and improving patient outcomes for these populations (hypertension, diabetes, asthma, obesity, alcoholism, etc.);

  • Developing patient education campaigns for identified health topics and ensuring all materials used are culturally, linguistically and literacy appropriate for patients and community groups;

  • Evaluating effectiveness of patient education campaigns and materials used by Promise staff;

  • Preparing presentations, conducting health screenings and doing patient outreach in the community, at businesses and industries, and for health fairs;

  • Meeting with health officials, school nurses and businesses to offer health education and health screening opportunities;

  • Becoming knowledgeable about the Title X (family planning) program, Iowa Breast and Cervical Cancer Program, Iowa Community Based Screening Services (CBSS), Susan G. Komen for a Cure breast cancer grant program, Family Crisis Centers victim advocate program, the range of sliding-fee scale services offered at Promise, along with other entitlement programs;

  • Referring patients to other programs and resources if and when needed;  

  • Interacting daily with team members at Promise (medical, dental, behavioral health, outreach, victim advocacy, etc.).


Qualifications include:

  • Associate's or bachelor's degree in nursing;

  • Bilingual language skills in English and Spanish is a plus;

  • Excellent customer service and communication skills;

  • Self-starter capable of building rapport with people, including diverse groups;

  • Cultural awareness and sensitivity;

  • Strong computer skills.
